The 5 forms below are required for all new patients.  You may print out each of the froms below and bring them with you to your appointment and avoid any paperwork in the office.  The only thing we will need when you arrive will be your insurance card and and personal ID/drivers liscence.

Appointments
Please give 24 hours notice if you are unable to keep a appointment. Other patients will appreciate your courtesy in releasing this time to them.

Insurance
Our office accepts most Kansas City area HMOs, a variety of PPOs, Medicare, and other health plans. Please call the number on your insurance card to verify that we are participating wiht your plan.
